The definitions of functionalities for basic and fully functional EHRs used above lack the precision that would allow systems to be tested for performance and certification. Lacking a certification program, physicians are correctly concerned that representations made by vendors may not be accurate, resulting in physician reluctance to commit to purchasing an EHR. To address this, the federal government provided initial funding for the Commission for the Certification of Health Information Technology (CCHIT), which began in 2006 to certify ambulatory and hospital EHRs for functionality,interoperability,and privacy and security. The certification criteria categories are listed in Table 1, and each category has 1 or more specific criteria. The CCHIT certification is achieved only if a system meets 100% of the certification criteria upon testing against patient-based test cases. At present, a CCHIT-certified vendor product provides a level of assurance that systems can perform to a uniform set of industry standards that have been designed by a broad base of stakeholders including users, vendors, and payers.
